Accessing Your Health Information
At Luton Hospital, we believe every patient has the right to access their health records, treatment history, and medical information in a safe, secure, and timely manner. Your health information helps you make informed decisions about your care and supports continuity in treatment.

Requests for proof of birth/death/admission
Luton's Patient Services Directory helps you navigate the hospital better. Inside you will find important hospital phone numbers, clinic locations, parking information, visiting hours, food services and much more.
Birth Certificate
The hospital does not issue birth certificates. After a birth, the hospital or midwife provides a Statement of Live Birth.
Death Certificate
The hospital does not issue death certificates. A Medical Certificate of Death is completed by the hospital and is sent to the funeral home with the deceased.
How You Can Access Your Information
Patients can access their health information by visiting our Medical Records Office with a valid ID and completing the Request for Health Information Form. They may also request encrypted electronic copies of their records through their registered email. A patient can authorize a family member or caregiver to access records by providing written consent along with valid identification for both the patient and the authorized person. Most requests are processed within 3–5 working days, with urgent cases given priority.
