Mouthwash can be a useful addition to your daily dental-care routine but does it matter whether you choose an alcohol-based or alcohol-free formula?

A fresh, clean feeling after rinsing is often what comes to mind when we think about mouthwash. With so many products available, one question commonly comes up: Should you choose an alcohol-based or alcohol-free mouthwash?

The answer is not simply that one is universally better than the other. The active ingredients, your oral-health needs and how you tolerate the product matter more than the presence or absence of alcohol alone.

Alcohol-Based versus Alcohol-Free Mouthwash

The most obvious difference is the presence of alcohol, usually ethanol, in the formulation. In alcohol-based mouthwashes, alcohol functions primarily as a solvent or delivery vehicle, helping keep certain ingredients such as essential oils in solution. It is therefore important not to assume that alcohol itself is responsible for all of a mouthwash's antimicrobial benefits. The formulation and active ingredients matter.

Alcohol-free mouthwash can also contain active ingredients such as cetylpyridinium chloride (CPC), fluoride or essential oils. Therefore, "alcohol-free" does not mean "less effective." Its effectiveness depends on what the formulation is designed to do.

Considerations when choosing a mouthwash?

Instead of choosing based solely on the words "alcohol" or "alcohol-free," consider what you want the mouthwash to achieve.

1.     For cavity prevention, consider a mouthwash containing fluoride, to help strengthen teeth against decay.

2.     For plaque and gingivitis, look for therapeutic formulations containing evidence-supported antimicrobial ingredients such as CPC or essential oils.

3.     For dry mouth, opt for an alcohol-free mouthwash or products specifically formulated to relieve dry-mouth symptoms.

4.     For bad breath, consider whether you need a product that merely freshens breath or one containing therapeutic ingredients that target the underlying causes of oral malodour.

5.     Dentist recommendations based on oral health needs.

When should you see a dentist?

Speak to a dental professional if you experience persistent toothache, bleeding or swollen gums, persistent bad breath, tooth sensitivity, persistent dry mouth, loose teeth and difficulty chewing or swallowing.  A dental professional can identify the underlying problem and recommend the most appropriate treatment.

Remember, both alcohol-based and alcohol-free mouthwashes have a place in oral healthcare. The question is “Which formulation is appropriate for my oral-health needs?”
